FYI Provider Registration

Welcome to the FYI Club registration process. This process will only take a few
minutes and is esential to ensure that we correctly present your business and
the services that you provide. Over the next few pages we will be prompting you
for such information as your business name, description of your services,
address, contact person, phone and email, web site, the areas where your
conduct business, business categories that you work, and you can even provide a
logo for your company that will appear throughout our web site.
Please read the vendor agreement below and check the check box at the end to
signify your agreement to get going.

FYI Club agrees to place an advertisement for Vendor on the FYI Club web site
in strict accordance with FYI Club’s size, format and content requirements. In
exchange, Vendor agrees as follows:
1. Vendor agrees to provide to each interested FYI Club member at least one
non-expiring coupon from Vendor, which will save the FYI Club member money off
the amount that the Vendor would otherwise charge for a service that Vendor
provides. The vendor agrees to honor the coupon/discount as long as it is
posted on the FYI Club, Inc web-site.
2. Vendor understands that Vendor’s competitors may also place advertisements
on the FYI Club web site if they enter into an agreement with FYI Club.
3. Vendor acknowledges that FYI Club members may post comments regarding
Vendor’s services on the FYI Club web site. Vendor, for itself and for its
owners and employees, agrees to release and hold harmless (i) FYI Club; (ii)
all of FYI Club’s officers, employees and stockholders; and (iii) all FYI Club
members from any legal actions, liability, expenses, losses or other damages
arising from comments appearing on the FYI Club web site. In the event that
Vendor violates this provision and brings legal action against FYI Club and/or
any of its officers, employees, stockholders or FYI Club members, Vendor will
not be entitled to any damages, and Vendor will pay all of the other parties’
attorneys fees and court costs.
4. FYI Club reserves the right to edit comments posted on its web site to
correct factual errors and to eliminate language that FYI Club, in its sole and
absolute discretion, determines to be inappropriate. These rights in no way
impose an obligation on FYI Club to ensure the accuracy of comments posted on
its web site nor do they limit the release set forth in Section 3 above.
5. Within 30 days after Vendor signs this Agreement, Vendor will provide FYI
Club with comments or testimonials of min of 2 to 5 of its customers for
inclusion on FYI Club’s web site. Vendor agrees to ensure that its owners,
officers, employees, contractors and their friends and relatives will not post
comments on the FYI Club web site regarding Vendor or any of its competitors.
6. FYI Club may terminate its web site at any time. If Vendor fails to comply
with any provision of this Agreement, FYI Club may remove Vendor’s
advertisement from its web site.
7. This Agreement is governed by Maryland law, and the parties agree that the
federal and state courts located in Maryland will have exclusive jurisdiction
over any matter arising hereunder.
